Please select the forms that pertain to your registration. Each team member MUST complete their own Participant form, and the club must complete the Member Facility Verification form.
The forms are all fillable on a computer, and then must be printed, signed, and emailed or faxed to the NOCA office one week prior to the start of the event in order for the team to be eligible to compete.

Adult or Coach Participant Form (18 year of age and older)
Youth Participant Form (17 years of age and under)
